On a very rare occasion if you haven't upgraded the chamber to metal, the figure 8 piece within the hopup where the slidestop goes thru can jam the chamber UP, pushed it up, so that the chamber jams the double horizontal grooves within the inside top of the slide.

If you have upgraded the chamber to metal, I advise you to take that piece out all together by unscrewing the 2 screws in the hopup, DON'T open the hop, just open very little just enough to slide that piece out. That "8" piece is to help pull down the chamber when cocking but it's really not needed if you have a steel chamber. Taking out this piece does not affect the accuracy of the gun.

Besides checking the o-ring, you should also check that the chamber, outer assembly is in good working order. Also check the pin that pokes thru the chamber's legs.

Also if your SV is double feeding, it's possible that when your slide went 3/4 forward, a bb is jammed at the ejection port. As suggested above, check for frags in the blowback unit.
